Alcoholic Hepatitis Mimicking Iron Overload Disorders With Hyperferritinemia and Severely Elevated Transferrin Saturation: A Case Report

چکیده
Iron overload disorders can present as non-specific symptoms and develop gradually but, if untreated, be very fatal. The common causes include multiple blood transfusions for chronic anemia increased iron absorption, including hereditary hemochromatosis (HH). HH is one of the usually presents with liver cirrhosis in a setting significantly elevated ferritin transferrin saturation. Alcoholic hepatitis clinical syndrome progressive inflammatory injury associated long-term heavy intake ethanol. However, patients alcohol abuse, excessive consumption disrupt metabolism releasing large amounts into circulation. This cause severely due to disruption metabolism, simulating such HH, especially patient also has cirrhosis. Even though high saturation greater than 45% recommended cutoff value sensitivity detecting disorders, it low specificity positive predictive often identifies people other acutely levels disease hepatitis. Recognizing this feature timely management spare from unnecessary phlebotomies prompt treatment alcoholic We an interesting case severe mimicking underlying

Hereditary hypotransferrinemia can lead to elevated transferrin saturation and, when associated to HFE or HAMP mutations, to iron overload.
As our understanding of iron metabolism improves through the more accurate description of iron metabolism actors, new causes of iron overload are identified. We, here, report 16 cases of hereditary hypotransferrinemia related to 4 previously undescribed TF (transferrin) mutations (p.Val221Gly, p.Arg609Trp, p.Glu370Lys, p.Tyr533X and p.Cys421Arg). We show that, besides increasing serum transferr...
متن کاملHyperferritinemia without iron overload in patients with bilateral cataracts: a case series
INTRODUCTION Hepatologists and internists often encounter patients with unexplained high serum ferritin concentration. After exclusion of hereditary hemochromatosis and hemosiderosis, rare disorders like hereditary hyperferritinemia cataract syndrome should be considered in the differential diagnosis. This autosomal dominant syndrome, that typically presents with juvenile bilateral cataracts, w...
متن کاملElevated Transferrin Saturation and Risk of Diabetes
OBJECTIVE We tested the hypothesis that elevated transferrin saturation is associated with an increased risk of any form of diabetes, as well as type 1 or type 2 diabetes separately. RESEARCH DESIGN AND METHODS We used two general population studies, The Copenhagen City Heart Study (CCHS, N = 9,121) and The Copenhagen General Population Study (CGPS, N = 24,195), as well as a 1:1 age- and sex-...
متن کاملElevated serum transferrin saturation and mortality.
BACKGROUND A large proportion of US adults have elevated transferrin saturation, an indicator of a predisposition for iron overload. The purpose of this study was to evaluate the relationship between elevated serum transferrin saturation and mortality. METHODS This cohort study was conducted using data from the First Health and Nutrition Examination Survey 1 (1971-1974) (NHANES I) merged with...

Isolated hyperferritinemia with normal transferrin saturation and dysmetabolism, in the absence of the two known mutations in the HFE gene of hereditary hemochromatosis Sir, We report on a patient who had isolated hyperfer-ritinemia with normal transferrin saturation, dysme-tabolism, and mild hepatic iron overload. He was negative for the two known mutations in the HFE gene of hereditary hemoch...
